Evaluation

Please check in 20 minutes before your scheduled appointment time to fill out our New Patient forms. Please bring your insurance information and current physical therapy orders from your physician. We are unable to treat you without these items.

Your therapist will assess you according to your diagnosed condition. This evaluation may include range of motion testing, strength testing, visual inspection, gait assessment and other special testing as indicated. Your therapist will develop a plan of care outlining your course of treatment and initiate a home program to assist you in recovery. Your first appointment will typically last just over an hour.

Other Considerations

Clothing / Footwear

If you are being evaluated for an upper extremity (shoulder or elbow), it’s helpful to wear a tank top to your appointment.  This allows your therapist to see your joint as it moves and also provides accessibility for any necessary joint and soft tissue work.

If you are being evaluated for a lower extremity (hip or knee), it’s helpful to wear shorts to your appointment. This allows your therapist to see your joint as it moves and also provides accessibility for any necessary joint and soft tissue work.

Please wear comfortable clothing that allows unrestricted movement. You may be doing standing, walking or jogging exercises.

Please wear appropriate, supportive shoes to your appointments. You may be doing standing, walking or jogging exercises.
